Q. When will my coverage take effect once my application is submitted?

A. Receiving your completed application in our office on or before the 20th day of the month will assure your benefits will begin the first of the following month.

Q. Will I get an ID card?

A. Yes, your membership card will be mailed to you on or before the first day of the month you become eligible. However, you do not need your card to receive care.

Q. Can I cover any family members other than my spouse and children?

A. The coverage under your plan is limited to your spouse and legally dependent children. Coverage for other family members can be provided through a separate plan.

Q. Can I cover my handicapped dependent child?

A. Yes, provided the child is incapable of self-sustaining employment by reason of developmental disability or physical handicap.

Q. What is a deductible?

A. It's a provision of an insurance policy that the first given number of dollars will not be reimbursed.

Q. If my plan is canceled will I have to pay the administrative fee again?

A. No, in most cases you would not be required to pay the administrative fee for reinstatement.

Q. My dentist is not on your list, how can he be added?

A. Please give us your doctor's name. We will contact him to see if he would qualify to join our program. You should also encourage your dentist to seriously evaluate our program.

Q. Which dentists on the list see children?

A. Most of the doctors are pleased to see and treat each member of your family. Please feel free to contact your dentist to schedule your child's first appointment.

Q. How long does it take to get an appointment?

A. It is not uncommon for a routine exam to be scheduled 4-8 weeks in advance. However, emergency services are always cared for immediately.

Q. What is the difference between DDS and DMD?

A. That is simply a designation as to the degree the dentist's receive from their dental school.
DDS = Doctor of Dental Surgery
DMD = Doctor of Medical Dentistry

Q. What is a co-payment?

A. A co-payment is the payment listed on the benefit schedule next to each procedure. The co-payment is the patient's fee for the procedure and is made directly to the dentist at the time services are rendered.

Q. How much of the co-payment does the Plan pay?

A. The co-payment is the only cost to the memeber for covered dental services performed .

Q. May I change the dentist I selected?

A. Yes, Simply contact and inform your local TDA with the new office you wish to go to.

Q. Will each member of my family get an I.D. card?

A. In order to control costs, there is one I.D. card sent to each family. You and/or your dependents do not need an I.D. card to receive care. (If the member insists, we can and will issue a second card as a courtesy.

Q. Can our company have a group plan if our employer doesn't want to pay any premium?

A. Yes. Your employer is not required to pay any portion of the premiums.

Q. If I want to change dentists, will the current dentist forward my records?

A. Yes, your dentist will usually forward pertinent dental records upon request from the new dental office.

Q. What does UCR mean?

A. Usual, Customary and Reasonable. This relates to the dentist's fees.

Q. Are there benefits for prescription drugs?

A. There are no benefits for prescription drugs, however we would advise you to submit any charges to your medical carrier for payment review.

Q. Are there any benefits if I need to go to the hospital?

A. There are no benefits for hospital care, however we would advise you to submit any charges to your medical carrier for payment review.
